Category/class: Amtiarrhythmic

Lidocaine Use:-It is local an aesthetic & a popular an-tiarrhythmicin intensive care units. The most prominent cardiac action is suppression of automaticity in ectopic foci.

Indication: Ventricular tachyarrhythmia’s.

Dosage:

 (*TD = 3 Times a day*)

(*OD = Once a day*)

(*BD = Twice a day*)

Adult dose:-

Ventricular Fibrillation

Initial dose: 50-100Mg/IV/OD

Ventricular Tachycardia

Initial dose: 50-100/mg/IV /OD

Cardiac Arrhythmia

Initial dose: 50-100mg/ IV/OD

Anesthesia

Local Injectable Anesthesia:-
Maximum individual dose: 4 mg/kg or 4.5 mg/kg

Maximum total dose: 300 mg




Pediatric Dose:-

Ventricular Fibrillation

Intravenous:
Initial dose: 1 Mg/kg IV/OD
Maintenance infusion: 20-50Mcg/kg/min/IV

Ventricular Tachycardia

Intravenous:
Initial dose: 1 mg/kg /IV/OD

Maintenance infusion: 20-50 Mcg/kg/min/ IV/OD

Route of Administration:

May be taken with or without food— Oral, IV,IM

 Frequency: Once a day (OD), twice a day / 2 Times a day (BD), 3 Times a day (TD)

Contraindications: Hypersensitivity to amide group of local anesthetics. A.V.block, cardiac decompensating, serious conduction disturbances, bradycardia.

Special Precautions: Renal or hepatic impair- Hypotension, ECG monitoring must during therapy.

Pregnancy Category: B (Safe breast feeding).

Lactation: Use with caution.



Side-effects: – Drowsiness, nausea, parenthesis, blurred vision, disorientation, twitching’s and fits.

Mechanism of action:-Lidocaine blocks both initiations and conduction of nerve impulse via sodium channel blockers.

Adverse Drug Reactions:-

Visual changes, ocular burning, Conjunctival hyperemia, diplopia, corneal epithelial changes.

General Considerations:-

Lidocaine Major Interactions:

Lidocaine+Arbutamine:-

These combinations of drugs composed can upset the rhythm of your heart. Consult your doctor if you have symptoms of chest tightness, irregular heartbeat, nausea or blurred vision.

Lidocaine+Conivaptan:-

Combination of these drugs may suggestively increase the blood levels and effects of lidocaine. This can increase the risk of severe side effects such as irregular heart rhythm and other cardiovascular harms. You may require a dose change or more recurrent observing by your physician to safely use both medicines.

Lidocaine+Dofetilide:-

Use dofetilide composed with lidocaine can raise the risk of an irregular heart rhythm that may be serious and potentially dangerous. If you experience of, lightheadedness, dizziness, shortness of breath, fainting.

Lidocaine+Tramadol:-

Tramadol may hardly reason seizures, and relating it with other medications that can also cause seizures such as lidocaine may increase that risk.  You may require a dose change or more recurrent observing by your physician to safely use both medicines. Lidocaine+Betaxolol:-
