Generic name: Procainamide

Brand name: Pronestyl

Category/class: Antiarrhythmic

Procainamide Use: It is an amide derivative of the local anesthetic procaine which was found to have antiarrhythmic activity but was not suitable clinically due to rapid hydrolysis and marked C.N.S. effects. Procainamide is an alternative drug to quinidine, having the same spectrum of efficacy. For I.V use it is safer than quinidine and has a more rapid action.




Indications: Premature ventricular contractions & tachycardia. Paroxysmal atrial tachycardia, Cardiac arrhythmias associated with anesthesia & surgery , atrial fibrillation.

Dosage:

 (*TD = 3 Times a day*)

(*OD = Once a day*)

(*BD = Twice a day*)

Adult Dose:

Arrhythmias:

Intravenous:-
Loading dose: 15-18 mg/kg/OD
Maintenance dose: 1-4 mg/kg/OD

Intramuscular:
50 mg/kg every 3-6 hours

Pediatric Dose:-

Arrhythmias

Less than 1 month:
Loading dose: 7 to 10 mg/kg/IV

1 year or older:
Oral: (available in Canada.)
15-50 mg/kg/day every 3-6 hours.

Intravenous:
loading dose: 3-6 mg/kg over 5 minutes
maintenance dose: 20-80 mcg/kg/minute
Intramuscular:
20-30 mg/kg/day 4-6 hours.

Route of Administration:

May be taken with or without food— Oral, IV,IM

Frequency: Once a day (OD), twice a day / 2 Times a day (BD), 3 Times a day (TD)

Contra-indications: Systemic lupus erythematous (SLE), A.V.block.

Special Precautions: Myasthenia gravis, don’t interact with sulphonamides, renal & hepatic dysfunction, cardiac failure.

Pediatrics’: Not recommended.

Pregnancy category: C Should be used with cautions.

Side-effects: Nausea, vomiting, weakness, mental confusion, hallucinations, flushing, hypotension, angioedema, agranulocytosis.

Mechanism of action: -Procainamide decrease myocardial excitability and conduction of velocity and depresses myocardial contractility by increasing the electrical stimulation threshold of ventricle, HIS Purkinje system, and through direct cardiac effects.



Adverse Drug Reactions:-

Increase in antiulcer antibodies, Diarrhea, Hypotension.
