Generic name: Propranolol
Brand name: Anol-20,Betabloc,Betapro, Betapro-SR, Ciplar,Carnol,Cardizole.
Category/class: Beta- blocker’s.
Propranolol Use: It decreases heart rate, force of contraction & cardiac output. On prolonged administration BP gradually falls in hypertensives but not in normotensives.
Indications:
Hypertension, Angina Pectoris, Cardiac Arrhythmias, Post Myocardial Infraction, Tachycardia, Migraine, Anxiety.
Doses:
(*TD = 3 Times a day*)
(*OD = Once a day*)
(*BD = Twice a day/2 times a day*)
Adult Dosage:-
Route of Administration:
May be taken with or without food— Oral, IV
Frequency: Once a day (OD), twice a day / 2 Times a day (BD), 3 Times a day (TD)
Pregnancy Category: C
Lactation:-
Move into breast milk use with alert.
Contra-indications: Chronic Obstructive Pulmonary Disease (COPD), Asthma, Partial or complete heart block. Acute myocardial infarction, Cardiogenic shock, Congestive heart failure.
Precautions : Impaired renal & hepatic functions pregnancy, cardiac failure. Myocardial insufficiency, reduced cardiac reserve.
Side-effects: Tiredness, lack of drive, G.I.upsets, nightmares forgetfulness, rarely hallucinations, muscle fatigue.
Mechanism of action:It is a selective beta-blocker that inhibits competition in response to beta1 and beta – adrenergic stimulation, thereby decreasing heart rate, myocardial contractility, BP and myocardial oxygen request. They reduce portal pressure by producing splanchnic vasoconstriction, thereby reducing portal blood flow.
Adverse drug reactions: Cardiogenic shock, Cognitive dysfunction, Bronchospasm, Angina, Arterial insufficiency, Hypotension, Bradycardia, Contact dermatitis, Hyperkalemia, Hyperlipidemia.
General Considerations:
»Advice Patient to take immediate-release tablets on empty stomach.
»Instruct caregiver to withhold dosing if child is vomiting or not feeding regularly.
»Counsel patient against sudden discontinuation of drug.
Monitoring Parameters:-
»ECG, Heart rate
»BP with IV administration
»Rena and Liver Function.
