Generic name: Sotalol

Brand name: Sotalar, Solet, Sotagard

Category/class: Antiarrhythmic

Sotalol Use: It is a non-selective B-adrenergic receptor blocker that also exhibits class Ill antiarrhythmic properties by its inhibition of potassium channels. Since of this double action, sotalol prolongs both the QR interval and the PR interval.

Indications: Ventricular arrhythmiass such as polymorphic VT, maintenance of normal sinus rhythm in atrial fibrillation/ atrial flutter.

Dosage:

(*TD = 3 Times a day*)

(*OD = Once a day*)

(*BD = Twice a day*)

Adult Dose:-

Atrial Fibrillation:-

Initial dose: 80Mg/Oral/BID
Maintenance dose: 240-320mg/Oral/OID

Atrial Flutter:-
Initial dose: 80mg/Oral/BID
Maintenance dose: 240-320Mg/Oral/OID

Ventricular Arrhythmia:-

Initial dose: 80Mg/Oral/BID
Maintenance dose: 240-320Mg/Oral/OID




Route of Administration:

May be taken with or without food— Oral, IV, IM

Frequency: Once a day (OD), twice a day / 2 Times a day (BD), 3 Times a day (TD)

Contraindications:  sinus bradycardia,unless a functioning pacemaker is present, Bronchial asthma,, second and third degree AV block,  uncontrolled failure and Hypersensitivity,cardiogenic shock.

Precautions: Pediatric: Safety not established

Pregnancy Category: – B Use with caution.

Lactation: Excreted in milk

Elderly: Use with caution

Side-effects: Torsade de pointes and other serious new ventricular arrhythmias, fatigue, bradycardia, vertigo, dyspnea, proarrhythmia, asthenia, dizziness, emotional liability, incoordination, paralysis, eosinophilia, leukopenia, photosensitivity reaction, fever, pulmonary edema, hyperlipidemia, myalgia, pruritus, thrombocytopenia, alopecia.

Mechanism of action: – Prolongs atrioventricular monophasic action potential, prolongs effective refractoriness of atrial muscle, ventricular muscle, and atrioventricular accessory pathways in both ante grade and directions via potassium channel blockade.

Adverse Drug Reactions:-

Bradycardia, Congestive heart failure, edema, visual disturbances, peripheral vascular disorders, extremely pain

Sotalol+Abarelix:-

Using sotalol composed with abarelix can increase the risk of an irregular heart rhythm that may be serious and potentially dangerous, while it is a comparatively occasional side effect. Sotalol+Abiraterone:-
